Hello, this is Four Seasons Plastic Surgery. ^^

When people start looking into their first double eyelid surgery, the first thing they usually do is search for photos of the line they like.

But even with the same line, the right surgical method and the line that can be achieved may differ depending on eyelid thickness, fat volume, eye-opening strength, and the structure of the inner eye corner.

Below, we've outlined the three things to check before your first surgery, the surgical methods, the criteria for choosing a line, and the schedule and precautions to prepare before your consultation.

Eye conditions we commonly check during consultations

Busan double eyelid surgery is performed to create a double eyelid crease. It differs in purpose from upper blepharoplasty, which corrects sagging eyelid skin and lax tissue together.

At the first consultation, we commonly check the following eye conditions.

Eyes where the eyelid covers the pupil

- The upper eyelid covers a large portion of the pupil.

Uneven eye size

- The two eyes differ in size, making the asymmetry visible.

Forehead straining

- You feel your forehead tense up when opening your eyes.

A double eyelid that appears and disappears

- The double eyelid crease keeps appearing and then fading away.

Three things to check before choosing a line

Skin thickness, fat volume, and the strength of the muscle that opens the eye all differ from person to person.

So rather than choosing a method based only on someone else's case or a trending line, it's important to first check the structure of your own eyes.

3 checks: lid thickness (thin: suture fixation; thick: fat/tissue trimming), eye-opening strength (ptosis correction), and epicanthal folds.

[Skin Thickness and Fat Volume]

If your skin is thin and there isn't much fat in the eyelid, the non-incision (suture) method, which creates the crease with thread, may be worth considering.

Conversely, if the skin is thick or there is a lot of fat, unnecessary fat and tissue need to be removed so the crease doesn't get buried or loosen. In that case, the incision method may be a better fit.

[Eye-Opening Strength]

If your eyes always look sleepy, or if your forehead wrinkles as you open your eyes, it's worth checking whether your eyelids lift sufficiently.

When the upper eyelid covers more of the iris than normal, it's called blepharoptosis (ptosis), and the most typical cause is weakness of the muscle that lifts the eyelid.

In these cases, double eyelid surgery alone may leave a sleepy impression, so we also consider eyelid-lift correction (ptosis correction), which adjusts the muscle that raises the eyelid.

[Inner Eye Corner Structure]

Whether there is an epicanthal fold covering the inner corner, along with eyelid thickness and overall eye structure, can change the shape and height of the line you can achieve.

This is why the same line photo can look different from person to person.

Rather than deciding which method is better between the non-incision and incision methods, the first step is to choose the method that suits your eye structure.

Insisting on a method that doesn't match your eye structure can lead to a loose line or revision surgery.

Busan double eyelid surgery methods: how they differ

The non-incision method creates the line by tying thread into knots and fixing them inside the eyelid.

It may be considered for those with thin skin, no sagging, and sufficient eye-opening strength, or for those whose double eyelid keeps appearing and disappearing.

The incision method removes fat and trims skin as needed while encouraging adhesion between the skin and muscle to form the line.

How much fat and skin to remove is determined by examining your eyes.

Consider both the "desired line" and the "achievable line"

Double eyelid lines are broadly divided into four types based on where the line starts at the inner corner of the eye and how its height continues toward the outer corner.

Double-eyelid lines: in (low, into the inner corner), in-out (widens outward), semi-out (starts just off the corner), out (even height).

Inline: A line that starts from the inner side of the eye and continues at a low height.

In-out line: A line that starts at the inner corner and gradually widens toward the outer corner.

Semi-out line: A line that starts slightly apart from the inner corner, sitting between the in-out line and the outline.

Outline: A line that starts clearly apart from the inner corner at a higher point and maintains a consistent height from the inner corner to the outer corner.

Even if you have a line photo you love, no single line suits every eye equally.

The shape and height of the line are determined by considering face shape, facial feature proportions, eyelid fat volume, and overall eye structure together.

Three things to prepare before your consultation

For a first surgery, checking the schedule and recovery process in advance is just as important as the method itself.

Make sure your important schedule doesn't overlap with your recovery period. Before your consultation, prepare the following three points.

1. Leave room in your schedule

If you have an important event you can't postpone, let us know during your consultation.

2. Hear about side effects and precautions

Make your decision only after fully discussing method-specific side effects, precautions for sedation anesthesia, and the recovery timeline during your consultation.

3. Only what you need right now

Rather than adding procedures like inner corner opening (epicanthoplasty) or eye-shape correction all at once just because someone else had them or they're trending, first check whether they're actually needed for your eyes right now.

Busan double eyelid surgery: frequently asked questions

Q. Does the non-incision method come loose?

Because the non-incision method fixes the line with thread, the crease may loosen over time.

For eyes with thick eyelids or a lot of fat, this possibility needs to be evaluated more carefully.

Q. Doesn't the line stay put with the incision method?

Just because it's an incision method doesn't mean the line is guaranteed not to loosen.

There are also cases where the line loosens or its height shifts, requiring revision surgery.

For eyes with thick skin or a lot of fat, the incision method may be a good fit since unnecessary fat and tissue can be removed at the same time. However, results can vary depending on eye structure and recovery progress.

Q. Can I bring a photo of the line I want?

Yes, you're welcome to show us a photo as a reference for the look you want. However, the same line can look different depending on whether you have an epicanthal fold, your eyelid thickness, and your eye structure.
